I had exactly this problem when setting up the Composite input on my PVR350 
card.

I'd successfully used the 'Tuner 1' input before and added a new channel to .
allow easy changing to the compostie input (Composite 3!?)

Once I'd got it working Ok, I couldn't get back to that capture card at all 
without the errors above.

IIRC I think my problem was caused by not having a value for externalcommand 
defined for the input in the cardinput table. I just set it to /bin/true and 
all is well now.

I think its the failure of MythTv to (in its opinion) successfully tune causes 
it.
