[mythtv] external channel change program

It seems as though the "external channel change" section of mythtv- 
setup is gone.

someone else on mythtv-users found the code, but its not being  
displayed in mythtv-setup.

someone else hacked their db manually.

I was trying to install .19 from atrpms last night and I had no way  
to enter in the "external channel changing" program (satcontrol et al).
