[mythtv] Ticket # 1043: Internal DVD Menu Support

Hello John,

Try this patch. It still has some issues, but i want to know if it plays
pirates of the carribean better then before.

patch was taken from mythtv root directory. I patch a file in
libs/libavcodec as well.
