[mythtv] [PATCH] add generic sql and commbreak query to protocol

This patch expands on the previous patch, and adds a specific commbreak query 
as well. 

Isaac, if this is going in the right direction and you approve, I would like 
to go on to add a few more extensions:

cutlist query
bookmark get/save
guide data retrieval
schedule recording

Just let me know.  I don't want to go to far down this path if this won't get 
included.

This patch sets the protocol version to 16.  If applied, mythweb would need to 
be immediately patched as well by one line.  I have attached that patch as 
well.
