Well, everything looks pretty good in your log, except this bit:

You didn't actually say what does or doesn't happen?  Does it just play 
silence at you, but the video works OK?  From the trace it would appear 
to be filling the soundcard and yet the soundcard is draining samples at 
a reasonable rate "to somewhere"...  Sure you don't have a second 
soundcard which it's writing to instead?
