[mythtv] [PATCH] add All/Unique option to Search Lists

This patch gives the user the option of only seeing unique titles when 
scheduling recordings using the Search List menu.  It only does this for 
Movies, Categories and Channels.

This works extremely well when looking for something new to record from 
PBS-HD, History channel, etc.  The "upcoming episodes" key is then 
useful to see other times that "title" is on.
