[mythtv] [PATCH] fix window border on Appearance change

	Small improvements to Jeremiah's latest patch.
1) Instead of grokking the window flags each time,
    add a bool does_fill_screen and base decisions on that
2) Improvement to the Qt/Mac 3.3.2 workaround -
    we move the window up a little bit to hide its titlebar
