[mythtv] [PATCH] GUI for setup pre-configuration questions

Currently the setup program requires both terminal input (it asks two 
questions on the command line) and graphical input (via a 
frontend-style window and menu).  The attached patch changes the two 
questions into checkboxes in a configuration wizard, so the user only 
has to deal with one method of interaction.
