[mythtv] [PATCH] Alsa audio sync and other fixes for latest big audio update

Ed,

Here is a patch against your latest posted audio update.  I took a clean 
checkout of mythtv as of Nov 2 and applied your audio patch to it.

This is a diff -Nura against it.

I have tested this and it has resolved the delayed audio for me.  I have 
a woodworking show recorded and you could really see the delay any time 
they used a hammer (it was hilarious).  Hammering looks good now ;-) .

The audio sync problem was in getBufferedOnCard, it was always returning 
the card buffer size.  There are miscellaneous other fixes and a few 
fixes for Alsa that seem to keep getting lost.
