[mythtv] [PATCH] iconmap extras

Contents of attachment:

master_iconmap.xml - better master icon map (more than just my lineup,
though significantly less than everyone's lineup)
mkiconmap.pl - patched mkiconmap.pl (new stuff needed for next attachment)
mkmastericonmap.pl - automatically retrieves preset lineups to build a
master icon map.
pruneiconmap.pl - after you've imported all lineups you run this to
"condense" them before exporting a new master icon map.
tv_grab_na_icons.patch - patch to add needed params to tv_grab_na_icons
(patch has been sent to author).

All of this should be a safe drop into contrib.
