[mythtv] [PATCH] DVB CAM: Cache PMT

Hi,

this patch implements caching of the program map table, which has to be 
sent to a CAM after a channel change. It makes the CAM start decrypting 
the channel a bit faster, so channel changes follow. It also adds the 
possibility of using a CAM on other cards than DVB-S.

It needs the following db update:
"ALTER TABLE dvb_channel ADD COLUMN pmtcache BLOB;"
