[mythtv] mythtv patch to use arts sound server

Here is a patch against current CVS that will let mythtv work with arts
(a sound server that kde uses).

Edit settings.pro to uncomment out the ARTS stuff and recompile.

Start mythfrontend and change the output device in Setup to ARTS:.

No more stopping artsd to run myth!

I have tested it with a local and remote backend.  It seems fine for me.

I have left a few extra things in the .h file because they may yet be
needed.  I'm not sure if the functions I left empty need something or
not.

Let me know if I need to change anything.
