[mythtv] [PATCH] bt8x8 card adjust filter auto-insert

Attached is a first attempt at autodetecting broken bt8x8 cards, and
applying the "adjust" filter on capture if necessary.  It tests for the
video_capability struct's "name" field starting with BT8?8, and sets a
flag indicating that correction is needed.  InitFilters checks that flag,
and adds an "adjust" filter to the head of the chain if the chain does
not already start with one.  It is assumed that for broken cards, the
adjustment should occur before any other operations, so the adjust filter
will still be added to the head of the chain if another one exists
somewhere else in the chain.
