[mythtv] Window caption patch

Here's a semi complete patch to add window caption changes to the frontend.  
This allows one to do fancy stuff with button mappings with irxevent.

Basically the caption will normally be "mythfrontend" while in the menus, 
"WatchingLiveTV" and "WatchingPreRecorded" while watching TV or a recording 
and  it will be the plugin name when a plugin is loaded (e.g. mythmusic).

I'm a real novice when it comes to C++ so it may well be possible to do this a 
better way...
