[mythtv] DB Error (blank frame list insert) and ERROR:file I/O problem in 'safe read'

My mythbackend has been having this trouble :
( I retyped it from the console output. Cant seem to be able to pipe it
to a file)


DB error (WriteRecordedToDB):
Query was:

The show above gets recorded and I can view it but when I try to fast
forward or rewind, mythfrontend crashes with the following error
message:

Error: file I/O problem in 'safe_read()'
:Bad addresss
Error: file I/O problem in 'safe_read()'
:Bad addresss
Error: file I/O problem in 'safe_read()'
:Bad addresss
Changing from WatchingPreRecorded to None
Segmentation fault

Anybody have any ideas as to what is causing this and how to fix it? 
This is my only major problem with mythtv .8. Otherwise it would be a
stable replacement for mythtv .7 for me.   I did not have this problem
in Myth .7.  I thought initially that it was an audio issue (it might
be, i dont know)  but even after a switch to btaudio for my tv wonder
pci and an upgrade to bttv and up2date all current redhat rpms,i still
get this problem.

 Also how can I make mythtv forget about the old recordings.  It
suppresses some recordings because they have been recorded before.  It
would be nice If i can turn this off on a per recording basis. If not I
would rather just turn it off altogether.
