[mythtv] Patch to fix mythvideo + clean up interface

Could you send your mythvideo-settings.txt file?  If you modified it at all 
from CVS, it didn't get included in the diff because of the name change..

Otherwise, looks good -- do I hear someone volunteering to maintain mythvideo?
