[mythtv] Sound and volume patch for MythTV

This patch incorporates support for adjusting the volume within
MythTV.  [ and ] decrease and increase the PCM volume, and | is the
Mute Toggle.

The mixer device and the initial master mixer and PCM volume can be
set through MythTV->Settings->Playback

This patch supercedes my earlier sound and volume patch, mostly
because this version is more configurable.
