[mythtv] [PATCH] HWSlice fixes version 2

Kenneth, another version of the hwslice fix here. This one will handle missing 
end slices nicely too.

The only thing it won't cope with at the moment is if you get slices from 
different frames but with increasing codes.... but if the signal is that bad 
then your pictures going to be totally screwed anyway! :-)

Note. The aspect ratio patch is in here too since I diffed against cvs.
