I've been using this patch with great success recently, but as a matter of 
principle I've been trying to find where the duplicate buffers come from.

There's nothing obvious (as I'm sure you already know) but I have noticed that 
  very occasionally I still get a future frame kicking in. This suggests to me 
that the buffer is getting duplicated when it is added to the "used" list, and 
is only later returned to the "available" list from there.

Still can't see where though! :-( My current favourite is some sort of problem 
with the inUseBuffers in avformatdecoder, but only because I understand it the 
least :-)

I'll keep looking...
