[mythtv] [PATCH] Transcoding, MPEG2, streaming

Okay,
    Here is a patch which (a) should get mpeg2 encodings working with the
    transcoder, and (b) includes working A/V sync code.  I have tested
    this to the best of my ability, and it seems to be working well now. 
    The mpeg2 problems were all trivial (and simply due to not having run
    through those code paths before).  The A/V sync code seems to play
    nice with mpeg2 and nuv streams.  I have noticed that I get better
    sync when using ffmpeg than mencoder.  I think this has something to
    do with the 'Skipping frame' messages from mencoder, since as far as
    I can tell, the A/V frames should be dead-on now.  Actually, I think
    I might be too agressive fixing sync issues (insert too many dupe
    frames, when audio is just slightly off).  That should just be a
    tuning issue though, and I could use other people's input on overall
    quality.

Isaac, I beleive this is working well enough that it can go in now.
