[mythtv] manual recording (much better)

Right now, R in the EPG is a shortcut for single record and
my fingers think that should be true in LiveTV also.

My question isn't so much how it should be called in the future
but now that it is in the code, how are users expected to
access it today (other than reverse engineering and hacking).
There is nothing in keys.txt and nothing in the unmodified
code that calls it. It's kind of an easter egg right now.

After I wrote that message, I realized that there are only
five items in Setup so I moved it to tv_settings.xml. It is
kind if a wizard page, isn't it? ;-)

In the future, I imagine it might make sense to have a
Schedule Recordings on the top that has a sub-menu for
Program Guide, Program Finder, Keyword Search, Manual
Recording, something that lists what is in the "record"
table that allows managing your preferences, and possibly
even Fix Conflicts. Without moving Fix Conflicts, that's
already five items just off the top of my head, three of
which already exist. MythWeb already has the record list
and keyword searches and it would be great if there were
similar features in the GUI.

An obvious enhancment for the Manual Record page would be
starttime and channel along with duration. You could use the
Start button to record now or pre-schedule shows to start early
and/or end late. I've seen this requested many times on this
list and know that I'd want to use it for sports and such.
