  http2: Myth/PutSetting allow '_GLOBAL_' to be used for the hostname

This makes PutSetting and GetSetting consistent in the use of _GLOBAL_ to
signify a global setting rather than a host setting.

You can still use an empty hostname in PutSetting for global settings.
