 Fixes for MythUI settings

 - Startup Database settings screen now uses MythUI settings
 - Change DatabaseSettings class to use MythUI.
 - Remove DatabaseGroupSetting which is a duplicate of DatabaseSettings
   class. Now uses the same class for both cases.
