 Convert MPEG2replex thread to QThread

 Refs #5501.  Convert the MPEG2replex thread from pthread to QThread.
 Additionally, convert the corresponding pthread_mutex and pthread_cond to
 QMutex and QWaitCondition respectively.  I also moved the thread into the
 MPEG2replex class with the mutex/condvar to make it easier to deal with.
