Log:

Improved stream change handling for blu ray playback.

This takes a fairly aggressive approach to stream changes by closing all
of the codecs and re-creating them. This shouldn't present a problem
though as stream changes should be tied very clearly to file/title
changes.

Fixes a few crashes when activating menu items.
