 Convert mythtranscode/mpeg2fix code from Qt3->Qt4 structures

 As Qt3 is long past in our history, it was high time to remove the uses of
 Q3PtrList, Q3PtrQueue from mpeg2fix.

 Also changed were some QMap and QString-related code that complained
 loudly
 when I removed qt3support from the .pro file.

 This could well fix a ticket as well.
