Log:

Fix the number of presentation surfaces used by VideoOutputVDPAU.

This replicates the previous behaviour of adding additional output
surfaces only when all other resources have been created. Should fix a
performance regression seen since the new VDPAU code was merged in from
the libmythui-osd branch.
