 Right now if there are CEA-608 or CEA-708 caption streams but no
 descriptors for them we ignore the CEA-708 streams and we allow selection
 of CC1 and CC3 CEA-608 streams. The attached monitors the actual caption
 packets and adds the streams to the menu even if there are no descriptors
 for them.

 This means CEA-608 CC2 and CC4 streams are selectable if present as well
 as all CEA-708 streams present. Also CEA-608 CC1 and CC3 will no longer be
 listed if there is no caption data available on those streams.

 There are no known problems with this patch, I just want to do some more
 testing and perhaps some code cleanup before I apply it.
