move HD PVR hacks to HD PVR specific code

remove for loops around ioctl calls, even for the hdpvr the only ioctls
were this makes sense is vidioc_encoder_cmd with START_STREAMING and
vidionc_g_fmt, since both fail if the device hasn't synced to the video
signal. for exapmle after a resolution change.
