 added patch (use 46) for fixing multichannel 2-6 in the same stream.
 sorry daniel I havent updated my source to your reviewed patchset yet. Any
 advice on how to merge easily instead of waiting for commit.
 The adjusted files are avfd.cpp in getframe, audiooutputbase.cpp and
 lavc(ac3dec.c dca.c liba52.c}
