Refs #3457. various updates for german EIT encoding fixups

Pro7/Sat.1 is switching their DVB-S transponder and the SI data
on the new transponder is now correctly declared.
Since DVB-C and DVB-T switched to the new transponder as source
thei SI data is now also correct. This changeset removes only
DVB-T Berlin and DVB-C from the fixup list, but I expect
the other DVB-T regions to do the same switch soon
(if it's not already done).

also removes DMAX from the fixup list and adds HSE/MonATV Berlin.
