Log:

Fixes #2873. Cleanups for MHEG (Interactive TV in UK) applies a patch from David Mathews with no edits.

 * This replaces asserts() with proper error handling.
 * This also a renames MHTemporary to the more descriptive MHUnimplementedAction.
 * There also are a few small rendering fixes.
