 Adds an thumbnail offset for creating initial thumbnail.jpg and the first
 chapter.  This will resolve the issue of blank frames being used.

 Set the following in mythburn.py to enable:

 thumboffset = (Time in Seconds)

 Also adds an option to use a bookmarks (only for myth recordings) as the
 initial thumbnail.jpg

 Set the following in mythburn.py to enable:

 usebookmark = True
