Comment:

 This patch is a rollback of the videout_ivtv.cpp code before bug #1236 was
 fixed.  There are then minor changes made, that do fix #1236.

 Everything seems to work fine (in terms of still leaving #1236 resolved)
 .. and the rollback removes the bugs that cause lockups and distortion in
 the guide, in this bug.

 I'm hoping to get this into 0.19, as these lockups seem quite serious to
 me.  Using the guide is not really possible with them. :/
