Log:

Added a setting to give programs broadcast in HD a priority boost.
Ordinarily, users should just give their digital inputs a boost to
cover all programs on those inputs.  In my current case, however, I
want SD programs to still use my analog inputs when possible.

Also added some more verbosity to the scheduler when -v schedule is
used.  Programs which can't be scheduled in pass 1 are now printed out
along with the already scheduled program which blocked it.
