Fix a bug in NuppelDecoder which would cause duplicate seektable/positionMap
	entries in the decoder.  This could really cause problems during seeking.

Add a few more VERBOSE VB_PLAYBACK messages to help squash any more bugs
	in the recent positionMap code consolidation.
